How much does production insurance usually run for a <$25,000 budget filming for 18 days over 4 months? Any recommendations of companies?

Too many factors to list, but suffice to say it depends on more than just your budget and schedule: Insurance needs to cover equipment, WC, L&D, cast, etc. and that coverage IS based on your schedule, but different projects have different requirements, so it depends on things like what camera package/grip-electric package you're using, how many crew you have, how many cast, what vehicles you're using during production, if you require any stunt work, animals, children... the list goes on... and on. The GOOD news, is that you can pull quotes from any insurer without having to sign on the dotted line until you find a package you're happy with. Good luck!

I recommend Film Emporium. Be prepared to answer the issues Shaun brought up. What you need to insure is even more important than the shooting days.
