This post is not to discuss this accident, it is to start a discussion on who is responsible for safety on a set and remind all to follow safety protocol when shooting. First I need to let everyone know that I am relatively new to industry. Couple of independent films, one major as department head, and some commercials. I am a member of IATSE 491. By my background and profession, Marine Coordinator/Water Safety, I look at safety on a set very carefully. As we all know above the line is supposed to provide a safe place to work...as we all know that doesn't always happen. My belief is it goes down the line...department heads need to look out for their and other crew, crew needs to look out for each other and we do need to look out for ourselves. Some us are older and that have seen more then others, need to be the voice of caution. On a union shoot there maybe more power behind the safety voice but lets look at other shooting going on. Shorts, Low budget Indie, Reality TV, Commercials My first real exposure to the business, I was part of reality show set in Costa Rica shooting for 2.5 months with no medic, no safety briefings,and this was a show that had 12 contestants and 16 crew off shore fishing. I have had reality shows come in town and shoot on and in the water with no real safety. I don't have any "real" answers but know we need to "Remember Sarah" while working on any type of project. I (you) maybe the only thing preventing another tragedy. mjn

I agree with having someone down the line, but other people on the track did not get injured. It was a freak accident that the piece of the bedding hit her onto the track. Had she been a little faster or a little slower it may not have happened. I am extremely sorry that she lost her life. But we cannot foresee where lightning strikes.

If we're shooting on a live track or airfield, we always make sure we have a direct contact with any controller, ideally with a chaperone from the facility concerned. To be ion a live railroad track without supervision seems damn scary to me.
That's what I was thinking. Can't believe a producer could be that stupid, or even someone else on the crew.
Angelina, I believe you now see the error of your original post but I will state that it was the producers and 1st AD's responsibility for the safety of the crew. It is actually very lucky that the director, crew and cast (William Hurt) was not killed in this accident.

Here in Australia no news about this has appeared anywhere, so you'll have to understand why I wanted to know more. I am a detective, mystery writer.
Angelina, this event happen in my town and I know many of the people involved. It is a mystery that keeps unraveling. I suggest you google the articles about it and see how the reports changed.

The producers, UPM, and 1st AD failed to do their jobs on this film. We have to keep safety as the priority when working in this industry. I believe that this accident will make larger productions safer then before, but worry about the low budget indie films, reality tv and documentaries. The very simple reality is, if you can't afford to do it safely, don't shoot the scene. No film is worth someone's life.

Hi Michael. We can do just about anything. Was there something you had in mind? We can print on the inside of the sticks (between the two sticks), I was thinking we could add a tribute to her there so that it would be in sight of all that use the slates.
